Waiver and Agreement
HEALTH & SAFETY REQUIREMENTS
In order to protect the health and safety of your pet, all of our other guests and staff, your pet SHOULD:
Be on a flea, tick, heartworm prevention medication program;
Have nails trimmed to a reasonable and otherwise safe length;
Be in good health.
Not be aggressive, toy/food protective (Remember: your pet will be around and/or interacting with other dogs and we must protect their health and safety).
Upon admission, pets must be free from any condition which could potentially jeopardize the health, safety, or well-being of other animals, our guests, clients, and staff; by utilizing our services and facilities the owner(s) represent, warrant, and certify that the pet being put into our care is in good health and have not been ill with a communicable condition in the last thirty (30) days.
If you are aware that your dog is not feeling well, please refrain from bringing them to daycare until he/she is feeling better. If your pet becomes sick while at daycare, you will be required to pick him/her up.
VACCINATION REQUIREMENTS
In order to protect the health and safety of your pet, all of our other guests and staff, your pet MUST have the following vaccinations:
*Rabies
Owners shall be required to submit proof of vaccination.
We strongly recommend the use of heartworm preventatives and the Bordetella shot.
Dismissals:
Your pet may be dismissed from daycare in the sole discretion of the staff for various reasons including, without limitation, aggression, excessive barking, illness, or other unsafe/unsanitary circumstances.
LIABILITY WAIVER & TREATMENT RELEASE
The Owner(s) understands, acknowledges, and agrees that our daycare services are provided in an environment where animals will be in proximity and/or interacting with one another. The Owner further understands, acknowledges, and agrees that, as a natural result of the foregoing, our daycare services carry with them an inherent risk and your pet may be injured in the course of interaction with other animals.
In the event that your pet becomes ill or is injured while attending daycare at our facility, we will make reasonable attempts to contact you. Should veterinary treatment be required, you authorize us to take your pet for veterinary care (whether to his/her own veterinarian or to a nearby veterinarian for emergency treatments). The Owner(s) shall be responsible for, and shall pay in-full at pick-up, all expenses for any veterinary treatment incurred by us.
In consideration of, and as material inducement for, our provision of animal daycare services, the terms and conditions hereof, and our obligations hereunder, the Owner(s), for himself/herself together with all heirs, successors and assigns hereby expressly and irrevocably releases, remises, and acquits The DogScape LLC, its members, managers, agents, employees, consultants, independent contractors, attorneys, advisors, successors, and assigns, jointly and severally, from any and all claims, demands, causes of action, obligations, damages or liabilities arising from or in connection with your pet’s attendance at our facility or our provision of services under this agreement.
I hereby certify that I am the actual owner of the pet for which this agreement (together with the waivers and releases contained herein) applies. I certify that I have carefully read and understand all of the statements contained in this agreement for the provision of animal daycare services.
